A call sheet is used to take out equipment needed for our filming production, usually the cinematographer or director will take out the equipment and complete the call sheet. The person who sign's the call sheet will be responsible for taking the equipment home and taking care of all the equipment until it must be returned to the media department. 

You complete this by writing down what equipment is needed eg. microphone, camera, SD card and Battery for camera. Then you write your details at the top of the sheet. This is information is needed, so if any equipment is missing when you bring it back, the media department is able to contact you straight away. 

At the bottom of the sheet, your media department leader will sign this off and also state a date you may bring back the equipment.
